COVID-19 Pre-Exam Questionnaire
During the COVID-19 pandemic, the Governor has restricted us to “curbside service” only. In order to best stream-line this process we request that you answer these few questions and either submit here, or print and bring them with your pet for your appointment. When you arrive for your appointment, please call from the parking lot and a team member will collect your pet. Once the doctor has read your concerns, examined your pet, and formulated a treatment plan, you will be contacted to discuss and set up a discharge time.
